To find eligible practice sites for our programs:

• Go to http://datawarehouse.hrsa.gov and look in the second section titled "Quick Access Reports" In the "Find Shortage Areas" click the link that says "HPSA and MUA/P by Address"
• Enter the facility address and click "submit"
