Facility Listings

The facility listings are updated semi-annually, to include facilities licensed by the Department of Public Health as of June 30th and December 31st of each year. Each listing includes an identification number, the facility name, address, county and license type. These listings are available in both Excel (.xlsx) and Portable Document (.pdf) formats. The Excel version allows the user to sort and subset the data. For a complete alphabetic listing by name, the PDF version is a smaller file which downloads quickly and is formatted to make reading and printing the listings easier.
