Individual Facility Reports

Listed below are links to find, view, and download the individual data reports that facilities submit to OSHPD. In most cases, you will enter the facility name or an OSHPD identification number into a search box to find an individual report file.

Annual Utilization Data

On an annual basis (calendar year), hospitals, long-term care (LTC) facilities, primary care clinics, specialty clinics, home health agencies, and hospices report facility-level data on services capacity, utilization, patients, and capital/equipment expenditures.

Annual Financial Disclosure Data

On an annual basis (individual facility fiscal year), facilities report detailed facility-level data on services capacity, utilization, patients, revenues and expenses by type and payer, balance sheet and income statement.

Hospital Quarterly Financial and Utilization Data

On a quarterly basis (every three months), individual hospitals and hospital systems report summary facility-level data on services capacity, revenues and expenses by payer, and utilization by payer.

Hospital Inpatient Discharge Data

On a semi-annual basis (every six months), hospitals submit abstracted information from individual patient records which include data on the patient’s ZIP code, gender, birthdate, race/ethnicity, and preferred language; source of, type of and status at admission; date of service; diagnoses, treatments/procedures, total charges, and expected source of payment. To protect individual patient confidentiality, data are aggregated into public use files and data products which are available on an annual basis.

Emergency Department Encounter Data

On a quarterly basis (every three months), licensed surgical clinics and hospitals licensed for outpatient surgeries submit abstracted information from individual patient records which include data on the patient’s ZIP code, gender, birthdate, and principal language, service date, diagnoses, cause of injury, treatments/procedures, and expected source of payment. To protect individual patient confidentiality, data are aggregated into public use files and data products which are available on an annual basis.

Ambulatory Surgery Encounter Data

On a quarterly basis (every three months), licensed surgical clinics and hospitals licensed for outpatient surgeries submit abstracted information from individual patient records which include data on the patient’s ZIP code, gender, birthdate, and principal language, service date, diagnoses, cause of injury, treatments/procedures, and expected source of payment.
