Formal Submissions
Facilities are required to formally submit their data and receive OSHPD approval by the report period due date.

Your data will be approved if it is at or below the established Error Tolerance Level (ETL), which we'll discuss in detail in Lesson 3. Once your data is approved, you can no longer correct it.

If your data does not meet the ETL, it will be rejected. If it's rejected, you must correct and resubmit it before the due date or apply for an extension. If you miss the due date, your data will be delinquent and your facility will be subject to penalties. (For details on extensions, penalties, and modifications, see current regulations.)

To increase the probability of your data being approved on your first formal data submission, you should test your data until it is at or below the ETL.
