The submission process is similar to what you have already seen in Lessons 1 and 2.

After you confirm the data type (discussed in Lesson 1), the contact information for your facility is displayed for verification. Ensure that the information is correct. If it is not, contact your facility's User Account Administrator (UAA) to make the necessary corrections to any information on this page.
